It is important that you add the Village, Town or City of birth not just the Country i.e. Greenwich, London not just London or Lagos, Nigeria and not just Nigeria. When completing the section for previous address and GP please include these even if this was outside of the UK. We appreciate that not every person would be registered with a Doctor in their Home Country, so please provide details of your previous Healthcare Provider (local Clinic or Hospital)
