It is important that you fill in all of the mandatory (*) questions as fully as possible including the village, borough, town or city of birth as well as the country and where it asks for previous home address and GP details, if this is outside the UK please still add this information, even if it was a local clinic or hospital and not a Doctor you saw for your healthcare needs, as it is another mandatory question and applications will be rejected without a fully completed request form for every patient wishing to register.

Two proofs of address (driving licence, bank/mortgage statement, tenancy agreement, HMRC/DWP letter, TV Licence or confirmation letter from place of education, utility bill but not mobile telephone or cable services, all dated within the preceding 3 months) and one form of identity (passport, birth certificate, photographic driving licence, emergency or armed forces staff badge or residence permit) PER PERSON if over 16 years old or birth certificate or passport and immunisation record if under 16 years; must be received within 3 working days of your registration form, either in person to the Practice during our opening hours or via the text message sent to your mobile or alternatively via the website, greccg.abbeypms@nhs.net. PATIENT DECLARATION for all patients who are not ordinarily resident in the UK

Anybody in England can register with a GP practice and receive free medical care from that practice. However, if you are not ‘ordinarily resident’ in the UK you may have to pay for NHS treatment outside of the GP practice. Being ordinarily resident broadly means living lawfully in the UK on a properly settled basis for the time being. In most cases, nationals of countries outside the European Economic Area must also have the status of ‘indefinite leave to remain’ in the UK.

Some services, such as diagnostic tests of suspected infectious diseases and any treatment of those diseases are free of charge to all people, while some groups who are not ordinarily resident here are exempt from all treatment charges.

NHS patient information sharing - Summary Care Record

NHS Zero Tolerance Policy

NHS ZERO TOLERANCE TO VIOLENCE I fully understand that the NHS is operating a permanent Zero Tolerance Policy (ZTP) towards violence and abusive behaviour (this also includes harassment, alarming, distressing, threatening, abusive, insulting, or racially prejudiced behaviour as well as violent behaviour) by any individual.
